Honestly, it sounds as if the perfect board for you is something like the Model F AT, or Apple's M0116. The AT can be easily modded to use ANSI or ISo layouts, and already has almost your entire wishlist of features. I've even seen a mod done with it to give it the standard 7u spacebar + Alt keys on...

An MF badge would be appropriate, I think. The DT-style looks really nice in the MF logo, it's simple enough, and recognizable. It could slot directly into the space where the oval IBM logos go in the older M's, or the rectangular space in the Unicomp/late Lexmark cases.

Just got a 64217 in the post. Very cheap on eBay, USB converted. The trackball is very smooth, and it works perfectly. Fabulously done! I love the way youdivided the case up. When I did mine, it was just the two pieces. What did you use to stick the parts together? I had used putty, but it didn't work as well as I had initially hoped. (I used Krazy glue to fill in my seam).

@micrex22: I think that the fidelity of IBM's printing makes it... alright. Looking at the blue-on-grey logo on my 1401, The middle bar on the M isn't fully cut through- only nicked in all three spots. I think the printing method would need to be improved/changed to get any better fidelity there.
