I've come across this thing in an eBay listing (photos from listing, not mine). Copyright is 1989, one year before Microspeed introduced the more ergonomic trackball design which is still being produced by CST.
Thank God they quickly changed this design.

I think you're supposed to slam your face on the desk and use your tongue to roll the ball. You use your nose to click the buttons. (...at least that's how I would do it)
Seriously though, as awkward and uncomfortable as this looks, I'm sure it's still much nicer to use than a standard hand-cramp mouse.
